The process of problem solving is the ability to fully understand and accurately define the problem. It is common for people to dive into projects, without an adequate understanding of the requirements and without performing the necessary background work. This can be the result of a pushy client, an ignorent manger or plain inexperience in dealing with problems of the specific nature.
The chances of solving a problem which is not understood, is similar to shooting in the dark. After several attempts, you may mange to hit the target, but it usually takes longer and more often than not will end up with unwanted bullet holes. In the case of the project, these unwanted bullet holes translate to a higher project cost, longer completion time, unsatisfied scope and poor quality.
